Eligibility: Children from birth to 18 years, who have a diagnosis of cancer, other life threatening illness, significant developmental disability, or debilitating medical condition

Required Documentation: Letter from child's physician confirming he/she meets eligibility criteria

Broome, Cayuga, Erie, Herkimer, Jefferson, Lewis, Monroe, Onondaga, Oswego, and St. Lawrence counties
