(Program)

Provides a congregate care setting serving older adults with psychiatric disabilities that works with residents on the skills needed to build independence.

Eligibility: Individuals ages 45 years or older with a primary psychiatric disability and one of the following: - SSI or SSDI related to a diagnosed mental illness - extended impairment in functioning due to mental illness - reliance on psychiatric treatment, rehabilitation, and supports

Application Process: Call for information and services

Required Documentation: Varies; call for required documentation

Fees: Covered by Medicaid; call for self-pay fee schedule
