Offers individual apartments in the community where participants learn basic living skills as they cook and clean for themselves as they transition to fully independent living.
Eligibility: Individuals with a serious psychiatric disability
Application Process: Appointment required; call for information
Required Documentation: Proof of mental health diagnosis, Single Point of Access (SPOA) referral form, physician's authorization
