Provides support and advocacy to chronically mentally ill adults who are homeless or housing vulnerable. Offers on-going support to assist individuals and families in maintaining permanent housing.
 

Eligibility: Individuals ages 23 to 60 years with a chronic mental illness who are homeless or housing vulnerable
