Independent Housing Program

(Program)

Provides housing support to eligible individuals with a serious mental illness diagnosis. Services may include housing advocacy, and housing search assistance, followed by furnishing, household supplies, and security deposit when necessary.

Eligibility: Individuals ages 18 years and older who: - have a primary diagnosis of serious mental illness - are in the process of relocating from a state-funded program (supervised site, hospital, correctional facility) - are experiencing substantial impairments in function - are homeless - have high Medicaid costs or are under an AOT order - have the capacity to live independently

Application Process: Complete a Single Point of Access form (SPOA) through Onondaga County
