HIV Housing & Support Services

(Program)

Guides and supports participants from conditions of illness, risk and dependence to conditions of wellness, safety and independence, while addressing the unique needs of their HIV diagnosis. Supports access to affordable housing options, case management, connection to substance use treatment, mental health programs, support with health care and medical compliance, and community resources, nutrition programs, and transportation.

Eligibility: Individuals or families diagnosed with HIV or AIDS
