Hospice Care

(Program)

Provides support to persons in the last stages of an incurable disease so that they may live as fully and as comfortably as possible. Care is provide by interdisciplinary professionals and specially trained volunteers to meet the needs that arise out of the crisis of terminal illness.

Eligibility: Individuals suffering from a terminal illness with a prognosis of six months or less
