Provides electronic equipment, namely, a button worn around the neck or wrist that an individual can press in times of an emergency.

Eligibility: Individuals who are susceptible to heart attacks, strokes, falls, or has a chronic medical condition

Application Process: Call for information and services

Required Documentation: Documentation of need

Fees: Call for fee information; often covered by Medicare or other insurance
