Center of Excellence for Alzheimer's Disease (CEAD)

(Program)

Provides diagnosis, assessment, ongoing care management, and education for individuals with Alzheimer's related dementia or suspected symptoms while working closely with family members, the patient's primary physician, and community agencies.

Eligibility: Individuals with Alzheimer's disease

Application Process: Appointment required; call for information

Required Documentation: Picture ID and proof of insurance

Fees: Call to verify insurance information

Broome, Cayuga, Chemung, Chenango, Cortland, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Otsego, Saint Lawrence, Seneca, Tioga, and Tompkins counties
