Integrated Outpatient Services Program

(Program)

Provides a full range of confidential counseling services including group and individual counseling, case management, vocational educational counseling, discharge planning, comprehensive evaluation and assessment, and referral services to other levels of care.

Eligibility: Individuals struggling with mental health and substance use disorders

Application Process: Call or walk-in for information and services

Required Documentation: Picture ID and proof of insurance or proof of income for sliding scale

Fees: Call to verify insurance information and sliding scale fees
