Provides transportation for ambulatory adults who are residents of the Towns of Delphi Falls, Fabius-Pompey, Lafayette, and Tully who are unable to drive themselves or arrange for other transportation to and from medical and dental appointments on Tuesdays and Thursdays.

Eligibility: Ambulatory adults who are unable to drive themselves or arrange for other transportation

Application Process: Call at least 3 days prior to scheduled medical appointment, at 315-696-8659; the dispatcher phone is open Monday through Thursday, 9 AM - 4 PM

Required Documentation: Signed release form prior to scheduling transportation

Fees: Clients are responsible for parking fees and should bring their handicapped parking tag if needed
