Co-Pay Assistance

(Program)

Provides financial assistance for patients suffering from certain blood cancer diagnoses to help defray the cost of insurance, co-payments, and premiums.

Eligibility: -Have a household income that is at or below 500% of the US federal poverty guidelines as adjusted by the Cost of Living Index (COLI), which is determined by zip code
-Be a United States citizen or permanent resident of the U.S. or Puerto Rico and be medically and financially qualified
-Have medical insurance and/or prescription insurance
-Have an LLS Co-Pay Assistance Program-covered blood cancer diagnosis confirmed by a doctor (see covered diagnoses listed above)
-Patient must be in active treatment, scheduled to begin treatment, or is being monitored by their doctor

Application Process: Call 877-557-2672 Monday - Friday, 8:30 AM - 5 PM or go online to apply

Required Documentation: Social Security and insurance cards
