Children and Youth with Special Health Care Needs

(Program)

Offers services for individuals from birth to 21 years of age who have or are suspected of having a serious or chronic condition and require health and related services of a type or amount beyond typically required by children. Assists with insurance access, setting up diagnostic testing and evaluations, offsetting medicals bills, and paying for hearing aids and other equipment. 

Eligibility: Individuals from birth to 21 years who have or are suspected of having a serious or chronic physical condition and require health and related services of a type or amount beyond that typically required of children
