Children and Youth with Special Health Care Needs Program (CYSHCN)

(Program)

Serves children from birth to 21 years who have a serious chronic physical, developmental, behavioral, or emotional condition by providing assistance in locating a physician or other health care provider, assistance in obtaining insurance and other programs that cover health care costs, and assistance in finding care coordination services and other support.

Eligibility: Families with children birth to 21 years who have a serious chronic physical, developmental, behavioral, or emotional condition

Application Process: Call or walk-in for information and services
