(Program)

Works in conjunction with Assertive Community Treatment (ACT) team (through Onondaga Case Management) to provide housing for individuals with chronic/severe mental illness. ACT provides all services/treatment and Helio Health provides a rent stipend.

Eligibility: Individuals with chronic/severe mental illness

Application Process: Appointment required; call for information

Required Documentation: Proof of mental health diagnosis, Single Point of Access (SPOA) referral form
