Supported Housing Initiative (SHI)

(Program)

Provides long-term case management and housing assistance for homeless/unstably housed individuals living with severe and persistent mental illness who need support in order to maintain independent housing in the community. Provides income-based, private apartments with a monthly rental stipend and case management services to assist with personal goals. 

Eligibility: Individuals ages 18 years and older who has a DSM V diagnosis by a psychiatrist or referred by an agency providing mental health services

Application Process: Referral from SPOA

Required Documentation: SPOA application; call for additional documentation
