Home Based Primary Care

(Program)

Provides health care services to eligible Veterans who are homebound. Utilizes a team of health professionals who work together to provide care to Veterans within the home.

Eligibility: Veterans who are homebound

Application Process: Referrals from primary care provider

Required Documentation: Call for required documentation

Fees: May be subject to a co-pay based on VA eligibility; call for information

Broome, Cayuga, Chenango, Cortland, Fulton, Hamilton, Herkimer, Jefferson, Lewis, Madison, Montgomery, Oneida, Onondaga, Oswego, St. Lawrence, Seneca, Tioga, Tompkins, and Wayne counties
