WIC Program (Special Supplemental Nutrition Program for Women, Infants, and Children)

(Program)

Provides nutrition screening, specific supplemental foods, and nutrition education to program participants. Eligible WIC participants include women who are pregnant or breast-feeding, infants from birth to 1 year, and children from 1 to 5 years of age.

Eligibility: Women who are pregnant or breastfeeding, or have infants and/or children under 5 years old residing in Jefferson or Lewis counties

Application Process: Appointment required; call for information

Required Documentation: Picture ID, proof of residency, proof of income, proof of participation in Medicaid, SNAP or TANF, immunization records for infants and children and/or confirmation of pregnancy

Fees: Call to verify insurance information and sliding scale fees
