Assertive Community Treatment (ACT)

(Program)

Due to Covid-19, Circare must limit face-to-face contacts to ensure the safety of clients and our staff. For referrals for any of their programs, please call first so inquiries can be directed and addressed appropriately.

Provides treatment and rehabilitation services to persons with a serious and persistent psychiatric disorder for whom traditional services are ineffective. Services are delivered through a multidisciplinary team.

Eligibility: Onondaga County residents ages 18 years and older with a diagnosis of schizophrenia, schizoaffective disorder, or bi-polar disorder with demonstration of a serious functional impairment

Application Process: Referrals through Onondaga County Single Point of Access (SPOA)
