Home and Community Based Services (HCBS)

(Program)

Due to Covid-19, Circare must limit face-to-face contacts to ensure the safety of clients and our staff. For referrals for any of their programs, please call first so inquiries can be directed and addressed appropriately.

Provides a variety of services for adults with mental illness and/or substance use disorders. Services include psychosocial rehabilitation, habilitation, family support and training, education support services, empowerment services, pre-vocational services, intensive supported employment, and ongoing supported employment.

Eligibility: Individuals ages 21 yeas and older with active Medicaid, enrolled in a Health and Recovery Plan (HARP) through a Managed Care Organization with persistent mental health diagnosis or substance use disorder
