Expanded in Home Services for the Elderly Program (EISEP)

(Program)

Provides assistance to help older adults ages 60 and older who are not Medicaid eligible and their families delay or avert costly and premature placement in nursing facilities. Services include case management services and non-medical supportive services which can include aide service for light housekeeping and personal care as well as respite. A consumer directly model, where the care receiver hires someone they know, is also offered.

Eligibility: Frail elders with chronic physical and/or mental impairments who are Onondaga County residents and not Medicaid eligible

Fees: Cost-sharing for aide service and day programming, depending on income; call for details
