Provides case management, in-home personal care, Telephone Lifeline, housekeeping, and day care subsidies for non-Medicaid eligible senior citizens

Eligibility: Individuals 60 years of age and older who need assistance with personal care and are not covered under Medicaid

Application Process: Referrals go through Onondaga County Office for Aging at 315-435-2362
