Environmental Modifications

(Program)

Provides consultation, project plan development, and cost estimates for projects such as building/repairing ramps, custom-made grab bars, bathroom and kitchen modifications, mounting systems for communication devices, home assessments, adaptations at work sites, and modifications at school.

Eligibility: Individuals with physical or developmental disabilities who are OPWDD eligible

Application Process: Referrals from Care Managers; call for information

Required Documentation: Documentation of disability and need for modifications

Fees: An hourly rate is charged to schools, businesses, or other organizations requesting consulting services; individuals may have the responsibility of paying for materials
