Self Direction

(Program)

Empowers individuals of all ages to customize their support services to meet their needs. Allows individuals to take control of decisions regarding what staff to hire, what services to receive, and how budget OPWDD funds.

Eligibility: Individuals who are OPWDD eligible and are enrolled in Medicaid and Care Management

Application Process: Referrals from Care Managers or circle of support members

Required Documentation: OPWDD eligibility letter, Self Direction approval from OPWDD and Life Plan; call for more information

Cayuga, Madison, Oneida, Onondaga, and Oswego counties
