School Based Health Centers (SBHC)

(Program)

Provides primary, dental, and behavioral health care to students in eight Oswego County schools.

 

Eligibility: Children attending APW Elementary, APW High School, Lura Sharp Elementary, Mexico Elementary school, Pulaski Middle-Senior school, Sandy Creek, and Fairgrieve Elementary schools

Application Process: Enrollment forms available on agency website, school nurse's office, or at the SBHC located in the school

Required Documentation: Immunization and medical records

Fees: No cost for visits at SBHC; there will be a charge for prescriptions, lab work, X-rays and specialty appointments
