Community Habilitation
(Program)
Offers skills training and habilitative supports within community settings for children and youth ages 10 to 25 years with a developmental disability. Skills include community safety, social awareness, financial management, communication, and literacy.
Eligibility: Children and youth ages 10 to 25 years who are OPWDD eligible and enrolled in Home and Community Based Medicaid waiver
Application Process: Referrals made by Medicaid Service Coordinators and the CNY Developmental Disabilities Regional Office
Required Documentation: Proof of OPWDD eligibility
