Provides a 24 hour, mobile, multi-disciplinary mental health team that works with individuals 18 years of age and older with a severe mental illness within their homes, neighborhoods, work environment, or any other place where issues may arise.
Eligibility: Individuals ages 18 years and older with severe mental illness
