Eligibility: Children in 9th through 12th grades residing in the Fulton City School District

Application Process: Visit the Central Registrar's office at the Fulton City School District Central Offices, 129 Curtis Street in Room 102 (inside the Fulton Junior High School)

Required Documentation: Original birth certificate, immunization records, proof of residency, parent/guardian driver's license or photo ID, custody papers (if applicable), latest report card or class schedule, copy of child's IEP or 504 plan (if applicable)
