Provides one-on-one assistance to help individuals enroll in government sponsored health insurance programs, such as Medicaid, Homecare Medicaid, Nursing home Medicard, and Medicare Saving Programs
Offers home visits as needed.

Application Process: Appointment required; call for information
Required Documentation: Proof of identity, proof of address, proof of income, copies of insurance cards, copies of medical bills, proof of resources
