Claxton-Hepburn Cardiac Rehabilitation

Uses diagnostic and monitoring equipment to test for and diagnose cardiological dysfunction. Offers medically supervised exercise, education and counseling for people who have had a heart attack, bypass surgery or other heart problems or procedures. Goal is to restore patients to their optimal level of health, physically, socially and emotionally.

Eligibility: Individuals in Saint Lawrence County recovering from or at risk for heart attack/condition

Application Process: Appointment required; call for information

Required Documentation: Physician referral and access to medical history

Fees: Call to verify insurance information; financial assistance available to those in need
