Jefferson County Home Health Care

(Program)

Provides multiple in-home services such as nursing, home health aides, physical therapy, occupational therapy, medical social worker, and case management. Provides mom and baby care, surgical care, elder care, and services for individuals who have experienced a stroke, or who have diabetes, heart disease, or breathing disorders. Offers a 24 hour on-call nurse to address patient issues, take referrals, and provide guidance. 

Eligibility: In-home services provided for Jefferson County residents who are: - elderly - have suffered a stroke - need post-surgical care - have breathing disorders, heart disease, or diabetes - pregnant/postpartum women and their babies

Application Process: Typically by physician referral; call or go online for information

Required Documentation: Documentation of eligibility needed

Fees: Covered by most insurances including Medicare and Medicaid; call for details
