Application Process: Registration forms can be downloaded and printed for parents to complete prior to registering in person
Required Documentation: Birth certificate, immunizations records, two proofs of residency, custody papers (if applicable), latest report card (if applicable), copy of child's IEP and/or 504 plan (if applicable)
