Case Management and Health Education

(Program)

Provides intensive case management services to individuals living with HIV/AIDS not engaged or sporadically engaged in care. Individual and group health education sessions review the importance of regular medical care, treatment adherence, and other health-related topics.

Eligibility: Individuals with HIV/AIDS who are uninsured or in need of services that are not covered by other third-party payers such as Medicaid or ADAP

Application Process: Can do walk-ins, but prefer to operate by appointment; call for information

Cayuga, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, and St. Lawrence counties
