Offers group day services to teach individuals to effectively use their daily living skills as active participants within their community.

Eligibility: Individuals with developmental disabilities who are OPWDD eligible

Application Process: Application form available online

Required Documentation: Birth certificate, Social Security card, Medicaid card, Medicare card, and any other insurance cards; Any of the following proof of disability (if applicable): Current ISP, current psychological assessment, notice of secision or documentation disability, copy of DDP2, DDP4 (MSC), copy of current LCED

Cayuga, Onondaga, Seneca, and Wayne counties
