Perinatal and Infant Community Health Collaborative

(Program)

Aims to improve perinatal and infant health outcomes including preterm birth, low birth weight, infant mortality, and maternal mortality by building systems of care for pregnant, post-partum women, and other pregnant individuals. Services include assistance in accessing healthcare and other support services, home visits, and education.
 

Eligibility: Pregnant and post-partum women/individuals residing in Oswego County and their infants up to 2 years of age and their families

Application Process: Call, walk-in, or go online for services; self- and provider-referral forms available online
