Nutrition Counseling

(Program)

Provides an outpatient medical nutrition therapy program designed for people living with chronic health conditions such as heart disease, diabetes, kidney disease, celiac disease, or other digestive diseases. Counsels expectant mothers and individuals who would like to lose weight, lower their cholesterol and blood pressures, or learn better eating habits.

Eligibility: Individuals with chronic health conditions, expectant mothers, or individuals who would like to lose weight, lower their cholesterol and blood pressures, or learn better eating habits

Application Process: Appointment required; call for information

Required Documentation: Physician prescription, physician referral (if required by insurance company), list of current medications and nutritional supplements, any pertinent lab test results, medical insurance information, any food diary/blood sugar records

Fees: Call to verify insurance information

Cayuga, Onondaga, Oswego, and Seneca counties
