Assists individuals with coordination needs such as goal setting, health promotion, transitional care, referrals to community and social supports and patient and family supports, and advocacy.

Eligibility: Individuals must: - receive Medicaid, Medicaid insurance plans, or be eligible to receive Medicaid - have been diagnosed with two or more chronic physical conditions, HIV/AIDS, or serious, persistent mental illness - have significant behavioral, medical, or social risk factors
