Offers low-dose computed tomography (CT) screening for adults who are current or former smokers.

Eligibility: Individuals who are ages 55 to 77 years and are current or former cigarette smokers of at least 1 pack/day for 30 years or 2 packs/day for 15

Application Process: Physician referral required

Required Documentation: Call for required documentation

Fees: Call to verify insurance information
