Children with Special Health Care Needs Program

(Program)

Provides assistance to children from birth to 21 years of age who have (or may have) a serious or chronic physical, developmental, behavioral, or emotional condition in locating a physician or other health care provider and may also provide assistance in obtaining insurance and/or other programs that help to cover health care costs. Supportive measures as well as medical information may be provided.

Eligibility: Children from birth to 21 years who have (or may have) a chronic physical, developmental, behavioral, or emotional condition
