Offers comfort care for people with terminal illnesses, enabling them to be cared for at home while still receiving relief from pain and other symptoms.
Eligibility: Lewis County residents with a life expectancy that is measured in terms of months with a "primary care person" (often a family member of friend) who can be available as needed
