Provides linkage to supportive housing and non-Medicaid care coordination for adults with serious persistent mental illness.
 

Eligibility: Individuals 18 years and older with a serious and persistent mental illness resulting in functional impairments/disability which seriously interfere with the ability to function independently for an extended period (at least 1 year)
