Diabetes Prevention Program

(Program)

Offers a lifestyle modification programs for adults diagnosed as pre-diabetic and/or those considered at high risk for pre-diabetes. Delivered over 16 weekly, one-hour sessions, follow up by 6 post core monthly sessions.

Eligibility: Individuals 18 years and older with a body mass index greater than 25, who have been diagnosed with prediabetes or are at high risk risk of developing type 2 diabetes

Application Process: Call to register
