Substance Use Care Coordination
(Program)
Coordinates care for individuals dealing with substance use, including providing connections to mental health, chemical dependency, and rehabilitation services, as well as primary care, counseling, support groups, and employment and food assistance.
Eligibility: Individuals at risk for becoming addicted, already addicted, or in recovery, and family members of those fighting addictions
