The Senior Health and Resource Partnership Project (Project SHARP)

Provides mental health and substanse use counseling referrals, aging assessments and services, and care facilitation along with other community based resources to adults 60 years of age and older to assist with their overall health and well-being.

Eligibility: Individuals ages 60 years and older in Onondaga County
