Early Intervention Program

(Program)

Provides a family centered program for early identification of infants and toddlers with disabilities. When possible, services are provided in the natural environment of the child.

Eligibility: Families with children age birth to three with developmental delays or a diagnosed condition with a high probability of developmental delay. Eligibility is determined by a multi-disciplinary evaluations and services are based upon individual child's needs.

Required Documentation: Picture ID and proof of insurance
